- Medicare, DVA, NDIS, My Aged Care or private health fund details
- Emergency contact and next-of-kin information
- Medical history, medications, allergies, and relevant test results or reports
- GP, specialist, or referrer details
- Session notes, exercise plans, assessments, and progress reports
- Payment and invoicing information
3. How We Collect Information
We may collect information directly from you when you:
- Complete our intake or consent forms (online or in person)
- Communicate with us via phone, email, or in person
- Are referred by your GP, specialist, NDIS provider, or other allied health professional
- Participate in assessments, consultations, or exercise sessions
In some cases, we may collect information from third parties such as healthcare providers, family members, or funding agencies—with your consent.

We collect and use your personal and health information to:
- Provide safe and effective exercise physiology and allied health services
- Communicate with you, your referrer, and your healthcare team
- Manage bookings, billing, and funding claims
- Monitor outcomes and adjust your program as needed
- Meet professional, legal, and ethical obligations
- Conduct internal quality improvement and clinical audits
We will not use your information for any other purpose without your consent unless required by law.
5. Disclosure of Information
Your information may be shared with:
- Your GP, medical specialists, and treating team
- Funding bodies such as Medicare, DVA, NDIS, My Aged Care, or insurers
- Administrative staff involved in scheduling or billing
- External professionals involved in your care (e.g., physiotherapists, dietitians)
We do not disclose your personal information to overseas recipients unless required for funding or administrative reasons and only in compliance with the APPs.
6. Data Storage and Security
All client information is stored securely in electronic and/or physical form:
- Electronic records are stored in password-protected systems such as Splose and Xero, both compliant with Australian data protection standards.
- Hard-copy records are kept in locked filing cabinets within restricted office areas.
- Only authorised staff members have access to client information.
- We maintain data security through encryption, secure backups, and regular staff training.
We retain records for the minimum legal period required by the Health Records Act 2001 (QLD) and other relevant laws, after which they are safely destroyed.
7. Access and Correction
You have the right to access or correct your personal information at any time. To do so, please contact us in writing. We may require proof of identity before releasing information. Requests will be handled within a reasonable timeframe.
If you believe any information we hold about you is inaccurate or incomplete, please let us know so we can update our records.
